Description of Forms / Requirements

Below is the list and description of forms and other requirements. All the forms and documents are enumerated but only those underlined are required for submission to the HAPAG-ASA Secretariat. Please click here for the summary of the forms to guide you on the usage.

1. Commitment of Implementation. Target areas, number of children to be fed and date of implementation. Needs to be submitted to Hapag-Asa Secretariat (Assisi Development Foundation) after the Diocese/NGO/LGU is trained depending on the decision to push thru with the program.

2. Memorandum of Agreement (MOA). Document formalizing the partnership of the diocese/NGO/LGU with Assisi Development Foundation (Hapag-Asa Secretariat). Should be submitted before Vitameal is released.

3. Form 1 – Barangay Profile. Document giving background on the site/barangay where the program will be implemented. The data may be gathered from the local Barangay Health Office, City Planning, Office Department of Education (DepEd), Day Care Center. Hapag-Asa Team has to coordinate with these offices to get the data.

4. Form 2 – Child Profile and Parents’ Consent. This is the document which supports the decision of parents for them and their child/children to be part of the program.

5. Form 3A – Master List of children. Listing of children intended to be fed. Submitted before a feeding program is launched. This will be the one evaluated in order to know the nutritional status of the kids. Please complete the data needed so as not to delay evaluation of your master lists. If indeed to be submitted through email, please accomplish the Excel Version since this is the format being used in the evaluation. To be accomplished by the Nutritional Assessment Team.

Note: For Pondo ng Pinoy Dioceses, master lists need to be submitted simultaneously to Pondo ng Pinoy Office and Hapag-Asa Secretariat.

6. Form 3B – Master List for Pregnant Women. If feeding of underweight pregnant women is intended, this is the form to be filled-up and submitted. Feeding of pregnant women is not compulsory. For easier and faster evaluation, you may also use the Excel Version. To be accomplished by the Nutritional Assessment Team.

7. Form 4 – Daily Attendance. Form which you may use for the recording of daily attendance of the beneficiaries of the supplemental feeding.

8. Form 5A – Weight Monitoring Form (for children). Submitted every month to the Hapag-Asa Secretariat so as to know the progress of the kids enrolled in the program. Please just indicate the exact month and day when the kids are weighed. To be accomplished by the Nutritional Assessment Team.

9. Form 5B – Weight Monitoring Form (for pregnant). If your beneficiaries for the feeding include underweight pregnant women, monthly weight is also needed to be monitored. This form will also have to be submitted monthly. To be accomplished by the Nutritional Assessment Team.

10. Form 6 – Schedule of Parents’ Classes. Form which may be used to record the schedule of the education classes, topics tackled and speakers.

11. Form 7 – Sick Referral Form. Form which may be used for referral of sick children included in the feeding to Health Centers or Doctors. Referral of sick children is needed to help the children sustain their progress in the feeding.

12. Form 8A – Volunteer Profile. Record of each volunteer which may serve as reference throughout the program and even for succeeding program implementation.

13. Form 8B – Volunteer Schedule. Schedules of all volunteers in the program including parents will have to be recorded. This form may be used for that particular purpose. Changes in the volunteers’ work and time assignments must also be reflected in the form for easier reference.

14. Form 9 – Inventory of Supplies. Form which may be used to track movement of supplies and other materials. This may be used as reference for purchasing of supplies which needs to be replenished.

15. Form 10 – Hapag-asa monitoring form. Serves as a means of updating each parish/site’s monthly status of program implementation. Please submit monthly to the Hapag-Asa Secretariat (one per parish/site/school) to know if some of the program's component needs further attention. To be accomplished by the Education and Support Services Team.

16. Form 11A1 – Diocesan Monthly Budget and Vitameal Request Form (for Pondo ng Pinoy Dioceses). Consolidated monthly budget and Vitameal request of each Pondo ng Pinoy Diocese. Needs to be submitted to Hapag-Asa Secretariat before the 15th of the month prior to the month being requested. To be accomplished by the Diocesan Coordinator.

17. Form 11A2 – Diocesan Monthly Budget and Vitameal Liquidation Form (for Pondo ng Pinoy Dioceses). Consolidated monthly budget and Vitameal liquidation of each Pondo ng Pinoy Diocese. Needs to be submitted to Hapag-Asa Secretariat before the 15th of the month along with the budget request. This will be the basis for the checking of the budget request. To be accomplished by the Diocesan Coordinator.

NOTE: You may also use the Excel Versions of both Form 11A.1 and Form 11A.2 for easier computation.

18. Form 11B – Vitameal Request and Usage Report (for other Dioceses/LGUs/NGOs). For non-Pondo ng Pinoy member partners, this form is the one to be accomplished and submitted to Hapag-Asa Secretariat for the Vitameal request. Report should be a consolidation of all the parishes/schools/sites being covered by the partner. To be accomplished by the Overall Coordinator.

19. Form 12 – Parish/Site/School Monthly Budget/Vitameal Request. Form to be accomplished by the Food Preparation Team of each parish/school/site. This is to be submitted to the Diocesan/Overall coordinator for basis of release to each area. This will also be the basis for the consolidated request to be sent to the Hapag-Asa Secretariat. Again, only parishes under Pondo ng Pinoy Member Dioceses will be given budget.

20. Form 13 – Terminal Report. Final summary report/documentation of the program implementation. To be submitted with the before and after photo of the children.

21. Other supplemental reports – School performance and attendance report. For partners implementing in the schools, report on the school performances / grades and attendance of the children before and after the feeding are also needed. This is to document the improvement on the mental development of the children since this report is relatively a more concrete basis of the benefits met through the program. Submitted to Hapag-Asa Secretariat together with the Terminal Report.

22. Other supplemental materials – Report Card. The report may be given to the parents of the children so that they may also track the progress or improvement of their children while in the program.
